Des ostéotomies trans-trochantériennes de rotation de la tête fémorale ont été proposées, pour soustraire la zone de nécrose à la zone d'appui maximum du cotyle et éviter ainsi la détérioration arthrosique : il s'agit soit d'ostéotomies de rotation antérieure (flexion) amenant la zone nécrotique vers l'avant, décrite par Sugioka, ou à l'inverse d'ostéotomies de rotation postérieure (extension), amenant cette zone vers l'arrière.
